Google expects pages to “be edited, reviewed, and updated on a regular basis” especially if they are for important issues like medical information, and states not all pages are held to such standards, but one can expect that Google wants information updated in a reasonable timescale. How reasonable this is, is dependent on the TOPIC and the PURPOSE of the web page RELATIVE to competing pages on the web.
You don’t have to be better than your competition at absolutely everything, so long as you identify enough points to build a keyword strategy around. For smaller companies, this means that you probably have to be better at the things the bigger fish haven’t thought of or aren’t actively looking to do. If you can’t think of anything at all then you have a much bigger problem than just coming up with keywords…

Understand and accept why Google ranks your competition above you – they are either: more relevant and more popular, more relevant and more reputable, better user experience or manipulating backlinks better than you.
